boredIt?s just about 2013 and I gotta say, I?m a little bored. At least, the blogger in me is. As an investor things are just peachy. All this panic about overpriced consumer startups has led to a nice softening of the market (periodic reports of Blubbles are great for that). And other sectors, like business to business, is still under capitalized v. the consumer sector.

Saracens coach kicks 'boring' tag into touch

On a dank day at Saracens' training ground, Mark McCall is throwing light on his team's try-scoring record, or should that be a lack of one. Saracens stand second in the Premiership table and are heading powerfully towards a fourth successive finish in the play-off places. Yet they have managed just 10 tries in 11 league matches, four fewer than the second and third bottom clubs, London Irish and London Welsh. It is an amazing statistic that has drawn accusations that Saracens, the 2011 Premiership champions, are boring.

That denunciation from, among others, two former England internationals turned BBC TV pundits, Jerry Guscott and Brian Moore, draws a passionate denial. "You could only say that's a factor if you've got an unhappy playing group," says McCall. "We've got a very happy, ambitious and motivated group. They know we don't set out to play in a boring way.

"They will tell you they don't feel shackled in any way. We have scored far too few tries in the Premiership. We'd love to score more. At the same time we have played some outstanding rugby."

Some context is called for. Saracens in the mid-Noughties were a mid-table team defined by a wretched tendency to lose matches from winning positions. They needed to change ? and keep changing. "We had a sobering experience about two-and-a-half years ago," says McCall. "There were consecutive games against Leinster [a 43-20 loss away] and Clermont [24-14 winners at Watford] that exposed a few things that had been disguised in the Premiership: our physicality and set-piece issues. It was a bit of a water-shed. Since then we have had 56 Premiership/Heineken Cup matches and we've won 43. That means we're doing something right."

And here's some more figures: today's meeting of Saracens and Northampton at Vicarage Road will be the seventh in the Premiership this season among last season's four play-off clubs. The previous six have produced just four tries. So the business end is being tightly fought.

Saracens, while hampered by back-row injuries, have built victories on a committed kick-chase and dynamic attack in the breakdown and set-piece. McCall's point is that they are not the only ones. "Harlequins understand that as well as having the ball-in-hand stuff, other things needed to be put in place. Their set-piece now is outstanding and their defence is very good.

"You can throw the try-scoring stat at us, I can throw a stat at you. We've kicked 54 penalties this season, which means the opposition have given away 54 penalties in their half. In the same number of games, Northampton have had [only] 25 penalties they've kicked. That is not the same as saying we have a win-at-all costs mentality. We're building pressure and the only way for the opposition to stop us is illegally. And when you have Charlie Hodgson and Owen Farrell, you kick your goals."

McCall says winning with one try or no tries away to Harlequins and Northampton, and against Gloucester at home, and 22-0 at Bath last weekend, was just fine. "At Sale and London Welsh we could have scored five tries, then we wouldn't be having this conversation," he says.

Conceivably, a shortage of tries may hurt Saracens if they finish level on points and wins with a rival. But Alex Goode's counterattack try at Franklin's Gardens and the one from a line-out by Richard Wigglesworth at The Rec proved they can be exciting and incisive. They are top of their Heineken Cup pool. And England have picked their backs liberally: Goode, Chris Ashton, Brad Barritt and Farrell started in the win over New Zealand.

And McCall promises Saracens will not stand still. "Kevin Sorrell [Andy Farrell's successor as backs coach] is making changes," he says. "Our first-phase moves have become more important. We tried before to pass around teams, using a pod system. Kev has made us a lot more confrontational with our attack, more tidy."

Italian Nobel scientist Montalcini dies at 103

ROME (Reuters) - Rita Levi Montalcini, joint winner of the Nobel Prize for Medicine and an Italian Senator for Life, died on Sunday at the age of 103, her family said.

The first Nobel laureate to reach 100 years of age, she won the prize in 1986 with American Stanley Cohen for their discovery of nerve growth factor (NGF), a protein that makes developing cells grow by stimulating surrounding nerve tissue.

Her research helped in the treatment of spinal cord injuries and has increased understanding of cardiovascular diseases, Alzheimer's and conditions such as dementia and autism.

One of twins born to a Jewish family in Turin in 1909, Montalcini was the oldest living recipient of the prize.

During World War Two, the Allies' bombing of Turin forced her to flee to the countryside where she established a mini-laboratory. She fled to Florence after the German invasion of Italy and lived in hiding there for a while, later working as a doctor in a refugee camp.

After the war she moved to St. Louis in the United States to work at Washington University, where she went on to make her groundbreaking NGF discoveries.

She also set up a research unit in Rome and in 1975 became the first woman to be made a full member of the Vatican's Pontifical Academy of Sciences in 1975. She won several other awards for her contributions to medical and scientific research.

Her face was instantly recognizable in Italy and she was well known as a dignified and respected intellectual, a counterbalance to the image of women succeeding through their looks and sexuality, exacerbated during the scandal-plagued era of former prime minister Silvio Berlusconi.

Two days after her birthday in April this year she posted a note on Facebook saying it was important never to give up on life or fall into mediocrity and passive resignation.

"I've lost a bit of sight, and a lot of hearing. At conferences I don't see the projections and I don't feel good. But I think more now than I did when I was 20. The body does what it wants. I am not the body, I am the mind," she said.

Italian Prime Minister Mario Monti said in a statement that Montalcini's Nobel prize had been an honor for Italy, and praised her efforts to encourage young people, especially women, to play a central role in scientific research.

Stock futures fall ahead of "cliff" talks restart

NEW YORK (Reuters) - Stock index futures fell on Friday, putting the benchmark S&P 500 index on track for a fifth straight decline as legislators prepared to resume talks to avoid the "fiscal cliff."

President Barack Obama and lawmakers are launching a last round of talks before a New Year's cliff deadline, when a series of tax hikes and spending cuts that could send the economy into a recession are set to begin.

U.S. stocks fell for a fourth straight session on Thursday but managed to recover most of their earlier losses after the House of Representatives, in the barest sign of progress, said it would return to Washington on Sunday night to work on avoiding the cliff.

Highlighting market sensitivity to cliff headlines, on Thursday stocks fell more than 1 percent earlier after Senate Majority Harry Reid warned a deal was unlikely before the deadline.

S&P 500 futures fell 4.9 points and were below fair value, a formula that evaluates pricing by taking into account interest rates, dividends and time to expiration on the contract. Dow Jones industrial average futures lost 40 points, and Nasdaq 100 futures dropped 11 points.

Economic data expected on Friday includes Chicago PMI for December at 9:45 a.m. (1445 GMT) while the National Association of Realtors issues Pending Home Sales for November at 10 a.m. (1500 GMT). Economists in a Reuters survey forecast a reading of 51 for the main PMI index and a 1 percent rise in pending home sales.

European shares edged lower as investors waited to see if a deal to avoid the U.S. "fiscal cliff" would be reached before further boosting their exposure to equities. <.eu/>

The yen fell to its lowest level in more than two years, lifting Japanese stocks to 21-month highs on expectations of drastic monetary easing, while shares in the rest of Asia rose as Washington races to avoid a fiscal crisis.

In a joint venture, Steelbridge Capital, a value focused real estate investor based in Miami, and a real estate investment fund managed and advised by affiliates of Apollo Global Real Estate Management, L.P., have acquired Concorde Centre II, a 10-story, 106,500-square-foot office building in Aventura, Fla.

?Office assets rarely trade in Aventura and they are a highly sought-after asset class,? Jay Caplin, Steelbridge Capital?s managing principal, told Commercial Property Executive. ?While Aventura is technically a suburban location, the nature of the area is very urban with a minimal supply of commercial and residential development land.?

The Class B building will be renamed Aventura View and undergo an expansive strategic capital improvement plan, expand property marketing and amenities, and introduce institutional quality property management and leasing.

?While the asset is in good condition and reasonably well leased, Steelbridge saw multiple value creation opportunities by bringing institutional?management?and leasing to the asset,? Caplin said. ?Aventura View?s location, combined with the fact that the average tenant size is smaller and many of the tenants have been at the property for many years,?generally?translates into a higher tenant retention rate.?

Located in the heart of Aventura?s business corridor at 2999 NE 191st St., the transaction marks the largest office purchase in the area in 2012.

Aventura View is?immediately?proximate?and visible to Aventura Mall, one of the top grossing malls in the country, and the area is characterized by dense vertical residential development.

?As a result, the office supply generally has not expanded significantly over multiple development cycles as it has always been very expensive to deliver new office product due to the cost of land,? Caplin said. ?With such strong barriers to entry, as the economy continues to improve and the supply of office product remains relatively constant, demand for office space will be directed to existing properties that are well located and positioned, such as Aventura View.?

Aventura View?is a rare property in that it has been owned by the same?partnership?since it was built in 1988. Steelbridge Capital identified this off-market property and the deal was negotiated principal to principal.

?Steelbridge is a value-oriented investor with a?special expertise in submarkets with high job growth and strong natural barriers to entry,? Caplin added. ?Our program targets the acquisition of real estate where the value has been temporarily discounted due to supply/demand imbalances in real estate capital or market fundamentals and which can be significantly enhanced through redevelopment and repositioning. By implementing a strategic and highly visible marketing campaign, in combination with a capital improvement program, Steelbridge envisions being able to create long -term value at Aventura View.?

It is anticipated that Steelbridge will implement a program that focuses on several major components of the building that will most improve the tenant experience including upgrades to the parking lot, improvements to the lobby and common areas, and some enhancements to the mechanical systems.

It?s obvious the Giants defense isn?t what it once was.

But if you ask their defensive coordinator why, he makes it clear it?s not the coaching.

?We?ve prepared better than what we?ve played,? Perry Fewell said, via Ralph Vacchiano of the New York Daily News. ?I think if you look at the tape we?re in position to make plays. We haven?t made plays.?

While coaches default into self-preservation mode this time of year, it?s hard to pin the blame for the Giants? precipitous slide on one man.

They were seventh in the league in total defense in 2010, and won a Super Bowl a year ago. This year, they?re 30th in total defense.

But mostly, they?ve been awful when it counted most, allowing 67 points in their last two must-win games, which leaves them only a slim conditional chance at the playoffs heading into the final weekend.

?We?ve been in position,? Fewell said. ?We haven?t made a football play for the last couple of weeks. We?ve been in position to make football plays, but it hasn?t gone our way.?

If there?s a sign that players know Fewell?s right, it?s that there?s no open or indirect grumbling about him yet.

?We won a Super Bowl in this system ? I think that?s the main thing,? linebacker Mathias Kiwanuka said. ?We had the same system that we did last year.?

?There?s a lot of support for Perry,? defensive end Osi Umenyiora added. ?I?ll just say that.?

Of course, the Giants were trending this direction defensively last year, finishing 27th in the league. But their play late in the year was much of the reason they won a title, so that fades from memory. That?s added some insulation for Fewell, and any offseason changes should rightly begin with personnel.

He?s had chances to interview for head coaching jobs in the past (and the Panthers may have made a significant mistake picking Ron Rivera over him), and will likely continue to. But until he gets one of those jobs, he?s left trying to scheme for an aging cast which needs a re-tooling.

For many years, Azerbaijani government has been showing care for the press and this benefits the whole society in general, Head of Azerbaijani Presidential Administration's Social and Political Department Ali Hasanov said.

He made the remarks at the award ceremony of the competition announced by Azerbaijani Fund of State Support to Media Development under the President on Thursday.

"The government has been taking care of the press for many years," Hasanov said.

Hasanov said that according to the concept adopted in this area, Azerbaijani Fund of State Support to Media Development under the President each year decides on certain issues related to labor and social problems of journalists.

"As a result of this attitude of President Ilham Aliyev to the press he was elected as friend of journalists. Next year, we will participate in the opening of the building, for the first time in the history of the country, built for journalists. Neediest and most productive journalists will be provided with apartments in that building," Hasanov said.

Significant events are to happen in Azerbaijan in 2013.

"Presidential elections are coming up in Azerbaijan. Work of media will increase during the year of presidential elections. The main issue is to satisfy the information needs of the Azerbaijani state and people by uniting, and to solve the existing problems in this area," he said.

Speaking of journalistic competition, Hasanov said that there are winners, but there are no losers in it, there are journalists who took the first, second, third and subsequent places.

"The first three places are awarded. Although others do not receive awards, they can participate in the competition next year, presenting even better articles. The main issue is that by holding these events, we demonstrate the care of the state and President Ilham Aliyev for the press," he said.

Nielsen reports that one in three social media users prefer social care to contacting a company by phone. That means that more consumers are using social media platforms like Facebook, Twitter, and the company blog to reach out to businesses and brands in the hope of resolving a customer service issue. SMBs are not immune to this trend, and many will see an influx of customer complaints through these channels, which can impact their online reputation. 2012 Review: Philanthropy, nonprofit stories focus on generosity ...

A community and its citizens can be distinguished in many ways but in no way better than the culture of giving, volunteering, service to good causes and needs of the neediest among us. When good people share their resources ? money, time, talent, expertise ? we are all enriched and we have the good examples we need to make us better. Our special section on Our Philanthropy and our special understanding of its importance, celebrates the giving-back in our community and the people who provide the front line services. As he approaches his fourth season as coach of the University of Kentucky basketball team, John Calipari sounds a lot like the awe-struck, wide-eyed kids who flock to his camps as worshipful followers. And the parents who tag along, hoping to get a noticing nod from the Great One.

?

He just doesn?t understand how he got so lucky.

?

He isn?t being coy. He means it.

?

For a kid from Pittsburgh?s Moon Township whose daddy worked the coal mines and whose grandparents came from the ?Old Country? speaking no English, the rarified air of the coach?s office at UK?s Craft Center is still a big deal.

?

?How could this happen? How could it happen to me?? he says, verbally pinching himself for a reality check. ?I wasn?t a great player, I didn?t go to a big school, I didn?t have the greatest coaching career. . .?

?

And yet, here he is, in arguably the best job in college basketball ? with the most devoted fans (or fanatics), with a legacy of great basketball, with a built-in magnet for attracting the best players and with the opportunity to ? well, win a national championship in your third year on the job. He gets it: The only place to go from that is to win the next one.

?

?We didn?t have much growing up,? he says of himself and his siblings, ?but our parents had expectations ? and that was that we could go to college. We all did, although our parents had only a high school education.?

?

It?s against that experience that he values education, though his frame of reference for what to do with a college education back then was limited. He thought he?d become a high school teacher because those were his role models.

?

He recalls what his mama told him: ?Don?t let your surroundings define you.?

?

That meant he couldn?t let himself be defined by the small, modest world in which he grew up but it also means he can?t let himself be defined by the bigger-than-life trappings around the storied head coaching job at the University of Kentucky.

?

?I am the coach, and that?s my job,? he says, pointing to a big sign painted on his wall, as if he might forget:

?

COACH YOUR TEAM.

?

?So, yes, I coach basketball, but if that?s all I do, I cheat the position.?

?

His upbringing, his mama?s advice, the influence of his wife, Ellen, his own values ? all these push him to do more than the minimum requirements of the job.

?

?I?ve been blessed. I want to set an example for giving back.?

?

And he has big plans for that, big dreams for making a difference, for being part of solutions and for playing a major role in the ?village? it takes to change the lives of children who face so many challenges in today?s world.

?

The kid who had nothing and now has plenty wants to help other kids dream big, to ?think beyond their surroundings,? to achieve beyond their own limited expectations.

?

In Memphis, his family?s last home, he and Ellen established a donor-advised fund at the Community Foundation of Greater Memphis (CFGM). It was, he said, an ?easy and clean? way to set aside charitable dollars for the community good, and it allowed the Caliparis to focus on the giving and not on infrastructure or the process or the details of management.

?

Through the Calipari Family Foundation for Children Fund at the CFGM, they could ask that their charitable donations go to qualified nonprofits, could respond to the CFGM?s recommendations, and could be sure all contributions were reported appropriately and met all legal standards. Primarily, they asked that their funds focus on ?children,? in the broadest sense.

?

In all, Calipari says the family has given a substantial amount through the CFGM fund. The grants have helped do such things as endow scholarships and support churches.

?

?Ellen and I consider giving a private matter; that is, we don?t give in order to draw attention to ourselves,? he said. ?In fact, one of the reasons we have given through the community foundation in Memphis is so we can quietly, anonymously help others.?

?

However, the Caliparis do sometimes choose to publicize their support of certain causes and organizations about which they are passionate. The reason for doing this, Calipari says, is to raise awareness, to encourage the public also to become active in supporting these same causes and organizations.

?

Lending their names publicly to charities often helps ?move the needle? for those groups ? which ultimately means more people will receive assistance.

?

Perhaps the most public of Calipari?s charitable efforts has been his pledge of $1 million, almost paid in full, to the Memphis Street Ministries a Christian ministry serving impoverished Memphis youth in what is the third poorest zip code in the nation.

?

Another large CFGM contribution was made to Team Focus, a charity that works with young men (ages 10-18) who don?t have a father figure in their lives. Specifically, the Caliparis? contribution has aided that group in establishing and maintaining a Lexington Team Focus chapter.

?

?We look for accountability ? and results,? Calipari said. ?We want to help make a difference.?

?

Now, the Caliparis are beginning to think about how they will deal with their philanthropy in the new home they have fully embraced. That doesn?t mean they?ll abandon Memphis ? they have special charitable interests there and expect to continue to support them. But their primary philanthropic focus will be Kentucky.

?

Since moving to Kentucky, they have given through their CFGM fund about $160,000 to various charities that assist Kentuckians. A majority of these groups are geared toward helping children, such as Team Focus, Christ the King School in Lexington and the School Choice Program in Louisville. Other Kentucky nonprofits that have already received support include The Jewish Community Federation of Louisville and the World Food Program USA.

?

The grants made through CFGM are in addition to contributions they have made from personal funds. These contributions are carefully vetted through Sandy Bell, UK?s compliance officer, to be sure there are no conflicts with NCAA rules.

?

In addition, he has used his ?Coach Cal? bully pulpit to leverage his ?sphere of influence? to raise funds through Hoops for Haiti, Kentucky Cares, and a Papa John?s campaign ($1 for every pizza purchased using the ?BOUNCE? code) for Children?s Hospital.

?

In Memphis he used his sphere of influence to assist the Jubilee Schools, eight schools and two urban education initiatives in Memphis, which opened in 1999 and now serves 1300 at-risk students. The Catholic-based schools, open also to non-Catholics and all ethnicities, outperform their counterparts on standardized tests in reading, math and science and provide a values-based environment.

He fully understands the advantage of the ?moral authority? of the UK basketball coach.

?

?About 98 percent of the money to the donor-advised fund was generated personally by me or my family,? Calipari said. This includes monies sent directly to the fund for his speaking engagements or personal appearances.

?

The first steps have been taken to establish a charitable presence in Kentucky. The Calipari Foundation, Inc. has been registered with the Secretary of State and the next step will be to apply for 501(c)3 status with the IRS ? that?s the tax-exempt designation required for public charities and foundations, making contributions to them tax-deductible to the extent defined by law.

?

Once the charitable status of the Calipari Foundation in Kentucky has been granted, he and Ellen will put some additional definition to its mission, make additions to the board of directors, and finalize the operational structure.

?

Calipari expects to be aggressive in giving-back ? and, at least initially, expects to focus on financial literacy for grade school children, a Catholic schools initiative (similar to Memphis? Jubilee Schools) ? because he believes every child should have the option of a values-based education ? and childhood obesity, which he knows is a serious problem in Kentucky.

?

He will also continue to hold himself accountable for being the right role model for the young men he coaches ? young men on track to lucrative professional careers where money can come early and aplenty. He believes he has a responsibility to help them understand the obligation to give back ? and to ?change the cycle of their lives.? Many of them have come from poverty, he says, and they have a chance to change themselves and their families.

Practice may elevate chance of death, second heart attack

PROVIDENCE, R.I. [Brown University] When heart attack patients present in the emergency department with some degree of anemia, or anemic patients have a heart attack, physicians have a tendency, but not much guidance, about whether to provide a blood transfusion. The idea is that a transfusion could help more oxygen get to the heart. Recent national guidelines suggested that there simply isn't good evidence to encourage or discourage the common practice, but a new meta-analysis of 10 studies involving more than 203,000 such patients comes down on the side of it increasing the risk of death.

The next step for determining when the practice could be appropriate needs rigorous randomized trials that will generate more decisive, high-quality data, said lead author Dr. Saurav Chatterjee, a cardiology fellow at the Alpert Medical School of Brown University and the Providence VA Medical Center.

For the analysis published Dec. 24 in the Archives of Internal Medicine, Chatterjee and his co-authors combined and analyzed data from studies in which anemia patients with heart attacks either received "liberal" transfusions or received more restricted versions of the treatment or no transfusions at all. Liberal transfusions were defined as cases in which patients either received two units of blood or more or had a transfusion even with a hematocrit reading (a measure of red blood cell concentration) higher than 30 percent (normal is in the low 40s).

What the researchers found, after statistical adjustments to control for important medical factors, was that the risk of death was 12 percent higher for people who received the liberal transfusions than those who did not. Moreover, the group that received liberal transfusions had twice the odds of having another heart attack.

"What we found is that the possibility of real harm exists with transfusion," Chatterjee said. "It is practiced in emergency departments all across the United States. I think it is high time that we need to answer the question definitively with a randomized trial."

Of the 10 papers that Chatterjee and his co-authors reviewed, all but one were observational studies. The only randomized trial was a small pilot experiment.

Searching for an answer

Chatterjee began the study when he was a resident at Maimonides Medical Center in New York. He noticed a paper by the AABB (formerly the American Association of Blood Banks) in which the association said there was not enough clinical evidence for or against transfusions in heart attack patients.

For clinicians, the practice has always been a tough judgment call. Some transfusions are clearly necessary, for example when a patient's troubles include not just a heart attack but also severe ongoing bleeding, Chatterjee said. But transfusions also create health risks, such as an increase in potential clotting because platelets may clump together more, or from an inflammatory immune response to the introduction of blood of a "foreign" source into the body.

Chatterjee and his co-authors decided to comb the literature to determine whether, if properly combined and analyzed, existing data could provide some insight. They found 729 potentially relevant studies, but only 10 that had the right data to help answer the question.

Few as they were, Chatterjee said, the studies all told much the same story.

"One of the things that struck us is that there were very few studies in evidence of transfusion at all," Chatterjee said. "In our case, though, we found that the effect was pretty consistently harmful across the spectrum of studies, spectrum of time, and spectrum of patients that were enrolled in the individual studies."

Chatterjee said the study should not be taken to mean that transfusions should be stopped altogether for anemic heart attack patients. Instead, he said, doctors must continue exercising their clinical judgment, at least until results from a large, well-designed randomized trial can be produced. Mindful of the risk his study found, however, they might just want to shift their thinking about where the border is among borderline cases.

"Before a definitive trial is out there, we should be conservative, especially considering the high risk of harm," he said.

[Brown University] When heart attack patients present in the emergency department with some degree of anemia, or anemic patients have a heart attack, physicians have a tendency, but not much guidance, about whether to provide a blood transfusion. The idea is that a transfusion could help more oxygen get to the heart. Recent national guidelines suggested that there simply isn't good evidence to encourage or discourage the common practice, but a new meta-analysis of 10 studies involving more than 203,000 such patients comes down on the side of it increasing the risk of death.

The next step for determining when the practice could be appropriate needs rigorous randomized trials that will generate more decisive, high-quality data, said lead author Dr. Saurav Chatterjee, a cardiology fellow at the Alpert Medical School of Brown University and the Providence VA Medical Center.

For the analysis published Dec. 24 in the Archives of Internal Medicine, Chatterjee and his co-authors combined and analyzed data from studies in which anemia patients with heart attacks either received "liberal" transfusions or received more restricted versions of the treatment or no transfusions at all. Liberal transfusions were defined as cases in which patients either received two units of blood or more or had a transfusion even with a hematocrit reading (a measure of red blood cell concentration) higher than 30 percent (normal is in the low 40s).

What the researchers found, after statistical adjustments to control for important medical factors, was that the risk of death was 12 percent higher for people who received the liberal transfusions than those who did not. Moreover, the group that received liberal transfusions had twice the odds of having another heart attack.

"What we found is that the possibility of real harm exists with transfusion," Chatterjee said. "It is practiced in emergency departments all across the United States. I think it is high time that we need to answer the question definitively with a randomized trial."

Of the 10 papers that Chatterjee and his co-authors reviewed, all but one were observational studies. The only randomized trial was a small pilot experiment.

Searching for an answer

Chatterjee began the study when he was a resident at Maimonides Medical Center in New York. He noticed a paper by the AABB (formerly the American Association of Blood Banks) in which the association said there was not enough clinical evidence for or against transfusions in heart attack patients.

For clinicians, the practice has always been a tough judgment call. Some transfusions are clearly necessary, for example when a patient's troubles include not just a heart attack but also severe ongoing bleeding, Chatterjee said. But transfusions also create health risks, such as an increase in potential clotting because platelets may clump together more, or from an inflammatory immune response to the introduction of blood of a "foreign" source into the body.

Chatterjee and his co-authors decided to comb the literature to determine whether, if properly combined and analyzed, existing data could provide some insight. They found 729 potentially relevant studies, but only 10 that had the right data to help answer the question.

Few as they were, Chatterjee said, the studies all told much the same story.

"One of the things that struck us is that there were very few studies in evidence of transfusion at all," Chatterjee said. "In our case, though, we found that the effect was pretty consistently harmful across the spectrum of studies, spectrum of time, and spectrum of patients that were enrolled in the individual studies."

Chatterjee said the study should not be taken to mean that transfusions should be stopped altogether for anemic heart attack patients. Instead, he said, doctors must continue exercising their clinical judgment, at least until results from a large, well-designed randomized trial can be produced. Mindful of the risk his study found, however, they might just want to shift their thinking about where the border is among borderline cases.

"Before a definitive trial is out there, we should be conservative, especially considering the high risk of harm," he said.

Anyone with constantly swollen, itchy, red and irritated earlobes could be allergic to nickel, and the solution may be as simple as allergy free earrings.

Nickel is a natural white metal that is often mixed with gold to create a less-expensive alloy. It is used in jewelry, such as earring posts, and in many other common items. Metal allergies are a type of skin reaction called contact dermatitis. Dermatologists are seeing more and more cases of metal allergies as body piercings have become more common. After poison ivy, metal allergies are the most common cause of contact dermatitis.

Most people won't have a problem the first time they wear earrings with nickel posts, but repeated exposure may provoke a reaction. Some individuals, however, are highly sensitive to nickel and will have an allergic reaction after just brief contact. An allergy sufferer may not realize which pair of earrings is causing the discomfort since it often takes a few days for the reaction to show up.

People of any age can suddenly get nickel allergies. Symptoms include swelling, itching and redness. In severe cases, blisters may form and eventually break, leaving ugly painful crusts on the earlobes. The skin can become leathery, cracked and dark. A doctor should be consulted if this happens; there is a risk of infection. Usually the reaction is confined to the earlobe, but some people are so sensitive that they can spread the reaction to another part of their body simply by touching it with fingers that touched the nickel. Sweating can make the allergy worse since moisture can provoke an electrochemical reaction between the metals in an alloy.

People who suspect they have a nickel allergy should consult a skin doctor, called a dermatologist. The dermatologist can confirm the allergy by performing a painless skin patch test. The doctor will apply several allergens to a small patch of skin on the back, using hypoallergenic tape. The patient must leave the tape in place for about two days, then return to the dermatologist. The doctor will remove the tape and check for a reaction. If the area is irritated, then the person is allergic to the substance the doctor applied there. There's no cure for a nickel allergy, but there are some things that will ease the pain. The dermatologist may prescribe a corticosteroid cream to be applied on the affected area to ease the itching and swelling.

Of course, the best route is to avoid exposure to nickel. Earrings with 14-carat gold, sterling silver or stainless steel posts are available. There are also plastic sheaths that cover the nickel alloy posts on earrings, as well as earrings with plastic posts. Earrings with titanium posts are another option. Avoid nickel in other objects, too. Some people have allergic reactions to the nickel in snaps and zippers on their clothing. Others have trouble with metal eyeglasses and sunglasses frames. People who are very sensitive should avoid eating foods that are rich in nickel, such as chocolate, grains and nuts.
